  BAYADA Home Health Care is currently seeking an experienced Physical Therapist Assistant, PTA, for a full time opportunity performing Outpatient home health visits for our senior living office, located in Moorestown, NJ. This office services adult and geriatric clients on a per visit basis in territories throughout Burlington County and Camden County, NJ. As a home care Physical Therapist Assistant, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.   • Make outpatient visits to clients in designated geographic territories.
  • Implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Participate with nursing in the multidisciplinary care plan, as appropriate.
  • Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet. Qualifications:
    • Have current licensure or certification in the state of NJ.
    • Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or
    • Have two years of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977
    • Have at least one year's work experience under the supervision of a qualified Physical Therapist.
